Our staff is available to assist you Monday-Thursday 8:00am-5:00pm and Fridays 8:00-2:00pm. Please note that our clinic days are Mondays and Thursdays, so calls and emails may not be answered until the following day.

Our clinic is located off of 10 Mile and I-84 at 875 S Vanguard Way. We are on the 1st floor of the 10 Mile Medical Plaza in Suite 110. Dr. Chlebeck operates at the Ten Mile Surgery Center and at St Luke's Nampa.

Our team is committed to answer any and all questions you may have. Prior to surgery you will receive a detailed packet with valuable information such as: items you will need before surgery and during your recovery period the best icing options: North Fork offers 2 products that can be picked up at your pre-op appointment setting up your post-op care setting up an online or in-person joints class

Please call our office to request a prescription refill. Please note that while we try to refill as quickly as possible, it can take up to 48 hours to be processed.

Please wait 3 months after your total joint surgery for routine dental procedures. After 3 months, you may resume dental appointments without antibiotic prophylaxis.
